What is a testimonial form?

A testimonial form is a short, guided form that asks a customer about their experience and captures the answer in a format you can publish: text, video, or audio. A good one asks open questions, collects the customer's name and details, and records their permission to use the testimonial before anything goes public.

The difference from a general survey is the goal. A survey measures satisfaction. A testimonial form collects one story you can put on your website, in the customer's own words.

What should a testimonial form ask?

A testimonial form should ask one main open question, a follow-up or two, the customer's details, and their consent. Keep it to what a happy customer can finish in two minutes:

  • A star rating (optional) as a quick first step.
  • One main question that pulls a story, like "What changed for you after working with us?" Customers answer in text, video, or audio.
  • Up to five follow-up questions, such as "What almost stopped you from buying?" Every follow-up you add is required, so two is usually plenty.
  • Their details: name and email always, plus job title, company, or website if you want them for credibility.
  • Consent: whether you may publish the testimonial, and where.
